ALL PATIENTS WILL BE REQUIRED TO WEAR A MASK AT ALL TIMES WHILE IN THE OFFICE.
We have a limited number of disposable masks but we can provide one to patients as needed.

If you or a member of your household has any of the following symptoms: cough, fever, shortness of breath, sore throat, muscle pain, loss of taste or smell, nausea, vomiting or diarrhea OR if you or someone in your household has tested positive for COVID-19 or been in close contact with someone that tested positive for COVID-19 in the past 14 days, please notify the front desk prior to your appointment. You will be contacted by a triage nurse with further instructions.
