We understand you may have many questions about All Care Medical’s upcoming transition to a Direct Primary Care (DPC) model, effective September 1, 2025. We hope this FAQ provides the clarity you need. Our goal is to continue providing you with excellent, personalized care.
- Q: What is happening at All Care Medical?
A: Starting September 1, 2025, All Care Medical will transition to a Direct Primary Care (DPC) model. This means we will no longer bill health insurance companies (including Medicare) for our services. Instead, patients will pay a periodic membership fee directly to our practice for a wide range of primary care services. - Q: Why is All Care Medical making this change?
A: Over the years, the complexities and restrictions imposed by insurance companies (including Medicare) – such as declining reimbursements, payment delays, service denials, and burdensome prior authorizations – have made it financially unsustainable to provide the personal, timely, and excellent care you deserve. This change allows us to focus entirely on patient care without these administrative burdens, fostering a stronger doctor-patient relationship. - Q: What is Direct Primary Care (DPC)?
A: Direct Primary Care (DPC) is an innovative healthcare model where patients pay their physician practice a periodic (e.g., monthly, semi-annually, or annually) membership fee. This fee covers most primary care services offered in the office, including routine check-ups, sick visits, some in-office procedures, and lab tests performed on-site. DPC practices typically offer enhanced access, longer appointment times, and a more direct relationship with your provider. - Q: When does this change take effect?
A: The new DPC model and membership fee structure will begin on September 1, 2025.
- Q: How much does the DPC membership cost?
A: The fee schedule is as follows:• Child under 18 years old: $2,000 per year
• Each additional child (same family, under 18): $1,500 per year
• Individuals 18-21 years old: $2,000 per year
• Individual over 21 years old: $3,000 per year
• Couples: $5,500 per year
• Family of 3 (Parents, 1 child under 21): $7,500 per year
• Family of 4 (Parents, 2 children under 21): $9,000 per year

- Q: How does the prorated fee for 2025 work?
A: For services from September 1, 2025, to December 31, 2025, you will pay a prorated fee, which is one-third (1/3) of the annual amount. This prorated amount must be paid before September 1, 2025, to secure your spot and receive care under the new model. The prorated amounts will change for partial enrollment periods. - Q: How are payments structured after the initial prorated period?
A: After the initial prorated period, you will receive a notice in December and June for one-half (1/2) of your annual membership fee. These payments will be due by January 1st and July 1st, respectively, to continue receiving care for the current year. - Q: Is there a registration fee to join the DPC practice?
A: No, there is no separate registration fee. However, you must sign up and pay your initial membership fee in advance to reserve your spot. - Q: Can I use my Health Savings Account (HSA) or Flexible Spending Account (FSA) to pay the membership fee?
A: Membership fees for DPC practices are typically not eligible for HSA/FSA reimbursement, but this can vary. We advise you to check with your HSA/FSA administrator or a tax advisor. You may be able to use these funds for services not covered by the membership fee (e.g., medications at wholesale cost, or services at outside facilities).
- Q: What services ARE included in the membership fee?
A: Your membership fee covers all services performed at our physical office, including but not limited to:
• Routine physical exams
• Pre-surgical physical exams
• Hospital follow-up visits
• PAP tests
• All routine visits, Telemedicine visits, unlimited sick visits
• Office surgeries and procedures (if appropriate in an office setting)
• Suture removal
• Telephone calls, Nursing calls
• Electrocardiogram (EKG)
• Pulmonary function test (PFT)
• Holter monitor (Heart monitor)
• Bone density testing as needed
• Lab work run in our office
• Required forms for school, sports, work, prior authorizations, etc.
• Appropriate referrals as needed
• Medication refills
• 24×7 telephone coverage for urgent issues
• Vaccines for patients under 19 years old
• Access to a wide variety of generic medications and supplements at wholesale cost (cost of medication is extra). - Q: Will I have any co-pays, deductibles, or co-insurance for services at All Care Medical once I’m a member?
A: No. For all the services listed above that are provided within our office, your membership fee covers them completely. There will be no additional co-pays, deductibles, or co-insurance billed by All Care Medical for these services. - Q: What services are NOT included in the membership fee?
A: The membership fee does NOT cover:
• Services provided outside of our office (e.g., specialist consultations, hospitalizations, emergency room visits, urgent care visits, surgeries performed at a hospital or surgical center).
• Labs sent to outside facilities (e.g., Quest, Labcorp – these will be billed to your insurance by the lab).
• Imaging studies (e.g., X-rays, CT scans, MRIs – these will be billed to your insurance by the imaging facility).
• Adult vaccines (patients 19 years and older will typically get these at their pharmacy, billed to their insurance).
• The cost of generic medications (though they are available to members at wholesale prices, which is an additional charge).
• Motor vehicle accident visits ($260 per visit, paid at time of service).
• Worker’s compensation visits ($260 per visit, paid at time of service). - Q: Are prescription medications included?
A: We offer a wide variety of generic medications at wholesale cost to our members. This means you pay our direct cost for these medications, which is often significantly lower than pharmacy prices. This is an additional cost to your membership fee, payable at the time of service. We will still send prescriptions to your preferred pharmacy if needed, especially for brand-name drugs or medications we don’t stock. - Q: Are vaccines included?
A: Vaccines for patients under 19 years old are included in the membership fee. Adult vaccines (for those 19 and older) are not included and will typically be obtained at your pharmacy, where they can be billed to your insurance. - Q: What about lab work?
A: Many common lab tests that can be run in our office are included in your membership fee. For more complex tests, or those we cannot perform on-site, we will draw your blood/collect specimens here if possible, but the samples will be sent to an outside lab (like Quest or Labcorp). The outside lab will then bill your insurance for their services as usual.
- Q: Do I still need health insurance if I join All Care Medical’s DPC practice?
A: YES, absolutely. Your DPC membership is NOT a health insurance plan. It covers primary care services provided at All Care Medical. You will still need health insurance for services outside our office, such as specialist care, hospitalizations, emergency room visits, urgent care, surgeries, complex imaging, and labs processed by external facilities. - Q: Will All Care Medical still bill my insurance after September 1, 2025?
A: No. As of September 1, 2025, All Care Medical will no longer participate with or bill any health insurance plans, including private insurance, marketplace plans, or Medicare. - Q: I have Medicare. Can I still be a patient at All Care Medical?
A: Yes, you can still be a patient by joining our DPC membership. However, All Care Medical will no longer bill Medicare for any services provided. You will pay the membership fee directly to us. You will still use your Medicare benefits for services outside of our office (specialists, hospitals, ER, etc.) as you normally would. You may be asked to sign a waiver acknowledging that neither you nor All Care Medical will bill Medicare for our services. - Q: What if I have a high-deductible health plan (HDHP)?
A: DPC can be a great complement to an HDHP. Your membership fee provides comprehensive primary care with no extra costs for in-office visits or procedures, helping you manage out-of-pocket expenses for primary care while your HDHP covers major medical events (after your deductible is met).
- Q: How do I join the new DPC practice at All Care Medical?
A: You can enroll by visiting our website at www.allcaremedical.org/dpc. There you will find payment options and can reserve your spot. For those without computer access, you can stop by our office to get a copy of the patient contract and register. - Q: Is there a limit to how many patients can join?
A: Yes, to ensure we can provide timely, personalized care with enhanced access, each provider will have a limited number of DPC patients. Registration is on a first-come, first-served basis. - Q: What is the deadline to enroll?
A: To ensure a spot and continuity of care, especially if you have appointments scheduled after September 1, 2025, you should complete your enrollment by August 31, 2025. - Q: What are the benefits of joining the DPC model at All Care Medical?
